If you have Medicare, your Annual Wellness Visit is a covered benefit designed to build or update a personalized prevention plan, separate from a standard physical exam. Sky Health welcomes new Medicare patients at our Voorhees office, with appointments typically available within the week.
Your Annual Wellness Visit typically includes a review of your medical and family history, current providers and medications, routine measurements, and a personalized screening schedule based on your age and risk factors.
An Annual Wellness Visit is not a hands-on physical exam. It's focused on prevention planning. If you also need a physical exam, this can often be discussed and arranged separately at the same visit or a follow-up.
Sky Health is accepting new Medicare patients, with same-week appointments typically available for both new and existing patients.
If you're a Medicare beneficiary who's had Part B for over 12 months, you're eligible for this visit once per year as a covered benefit. It's a good fit whether you're new to Sky Health or a returning patient due for your annual check-in.

Before your visit, you'll receive new patient forms through the healow Patient Portal to complete at your convenience. Please also bring a list of your current healthcare providers, medications, and any recent health changes so our care team has the most up-to-date information.
You'll leave with a personalized screening schedule for the year ahead.
